In order for your visit to go as smoothly as possible, please review the information below. Please arrive at least fifteen minutes before your appointment if you are a new patient or you have any changes to your insurance or personal information.

How to Make an Appointment

Make An Appointment at Check-Out

While we have same day availability, we cannot always guarantee you an appointment at the day and time you want. Ensure that you can get your appointment when you need it by scheduling a follow-up before you leave the office. Occasionally, your primary care physician may make an appointment for you. We will send you a reminder call two days before your visit to remind you, but if you need to change the day or time, please call us.

Financial Policies & Payment

No Shows

A $50 fee applies to missed medical appointments and cosmetic appointments. If you are unable to attend your scheduled visit, please contact our office prior to your appointment time. We are happy to help reschedule and avoid a no-show charge when notice is provided.

Cancellation Policy

If you are unable to keep your appointment, we ask that you cancel at least 48 hours in advance. Appointments canceled with less than 48 hours’ notice may be subject to a no-show fee. Advance notice allows us to offer the appointment time to another patient.

Payment

As required by your insurance plan, payment is collected at the time of your visit. Patients paying out-of-pocket for medical or cosmetic services are expected to pay on the date services are rendered. For your convenience, we accept cash, check, credit cards, and CareCredit.

Refund/Return Policy

We make every effort to ensure billing statements are accurate and clearly explained. If an incorrect charge occurs, the overpaid amount will be promptly refunded. Refunds apply only to billing errors. To review our Privacy Policy, please click here.

Referrals

Patients whose insurance company requires a referral to our office must bring a copy of the referral on or before the day of their appointment. Patients presenting without a valid referral on the day of their visit will be asked to reschedule. Unfortunately, we will be unable to assist you in obtaining a referral at the time of your visit.
